We currently do not have online payment options. We do have the option to send you a secure message with a secured payment link. Otherwise you can make payments over the phone, in person or through the mail.

We encourage new patients to come ready for their appointment.  

Print your Health History form and complete it at your convenience. Then bring it with you to your first visit.
